boards/sodaq-*: prefer default ADC config from Arduino

Right now the gain and reference are hard coded. Until it is configurable
at runtime prefer the same default as with Arduino Core. That is,
GAIN_DIV2 and REFSEL_INTVCC1. This way we can properly measure the
battery voltage.

This is still currently a hack to hardcode it as the value can be deduced
from the `BOARD_MODULE` daughter board name.
But it requires more cleanup and could come in a separate step.
Part of moving CPU/CPU_MODEL definition to Makefile.features to have it
available before Makefile.include.

boards/common/arduino-atmega: remove special case for jiminy-mega256rfr2

The board is deprecated, no need to ignore this board anymore here.
Having the `arduino` and `periph_pwm` features is now required for all
boards using 'arduino-atmega' (as it was except for that board).
If this should change in the future, it should be defined either in each
arduino board, in another board common, or per CPU_MODEL.

cpu/sam0: refactor ADC_0 into plain ADC

This change is for all boards with a sam0 cpu. This cpu just has one ADC.
It is unnecessary to have defines with ADC_0_ prefix as if multiple ADCs
are possible.
Some defines were not used, such as ADC_0_EN, ADC_0_CHANNELS,
ADC_MAX_CHANNELS, ADC_0_CLK_SOURCE, ADC_0_CHANNELS
Change all ADC_0_ prefixes to ADC_

Enable the handling of flashing `softdevice.hex` when flashing the firmware
for openocd.
However, for flashing, only the `hexfile` and `binfile` can currently be used.
The `elffile` is generated with local pages aligned to `0x10000` which makes
the program starting at `0x1f000` be flashed from `0x10000` with padding bytes
even if the `.text` section is indeed at `0x1f000`:

The padding bytes would go through `verify_image` in `openocd` so be expected
to not be overwritten but are by `softdevice.hex`
Using --nmagic at link time removes the local page alignement but would

- No one seems to use it

- It has a significant user base
- It is an off-the-shelf product, so it might be easily obtained for testing

'reset_config connect_assert_srst' when flashing and resetting only.

When in an invalid state, the board could not flash anymore.
Like for example after flashing `tests/driver_adt7310` this makes it
flash again.
I used `reset_config srst_only` as the other kinetis boards are also

'openocd/openocd-adapters/dap.inc.mk' to select the adapter.
Using 'OPENOCD_EXTRA_INIT' for this was deprecated.
When 'DEBUG_ADAPTER_ID' was set it was already the case but not with
'SERIAL'.
The compatibility with using 'SERIAL' is maintained.

pkg/nordic_softdevice_ble: reset memory in the .hex file
...
softdevice needs the memory at 0x2000 to be initialized to 0xffffffff
according to #5893 and testing. However, the addresses [0x8bc, 0x3000[ are not
set in softdevice.hex.
So use a modified hex file with all the memory set to 0xff as it is the rom
reset value anyway.
This change updates the `.hex` file instead on relying on erasing the
memory.

murdock: nordic_softdevice_ble: softdevice.hex is a test input
...
When running tests using 'nordic_softdevice_ble', the 'softdevice.hex'
file must also be taken into account for the test hashing and be
uploaded to the separated murdock testing boards.
When listed as dependency from `test-input-hash` the file must have a
target, which he has not. The file is implicitly created when compiling
`ELFFILE` so declare it as order only dependency.
In practice `BASELIBS` could be enough or even `pkg-build-softdevice`
but I do not want to be depend that much on internals there.

64 KiB flash) instead of the use 64 KiB flash (secretly coming with 128 KiB)

The MSB-A2 and the Avsextrem board once had support for using the USB
interface of the LPC2387. The code setting up the USB clock has been unused
for ages and is now defunct, as the required value for `USBCLKDivValue` is no
longer present in RIOT's code base. This commits removes the defunct and unused
code.

The mips-malta board is a maintainance burden, has no working UART input
and is unobtainable and thus must be removed.
1. Unobtainable board
=====================
The mips-malta board is not an off-the-shelf part. A quick web
search only show the MIPS website where one is told to "contact sales".
I could find it on ebay, used, at €155 and from single seller.
Not having access to the board means:
a. We cannot maintain it. In fact it could be broken right now.
b. Potential RIOT uses have not access to the board either. In other
words, it is pointless to run on hardware nobody has.
2. No working UART input
========================
Not all applications need UART input, but that is no excuse for not supporting
it:
a. Makes development & debugging way harder.
b. It is impossible to run interactive tests.
b.1. Constrains the rest of the platforms by providing an incentive to not
make tests interactive.
c. The lack of UART is a witness to the poor quality of the port.
I want to stress point (c). If something as basic as a serial port cannot work,
how can we expect more complex fucntionality to work. The answer is impossible
to know, because of point (1).
3. Maintainance burden
======================
The RIOT project has limited time and human resources which can be better spent.
a. Compiling for mips-malta wastes CPU time.
b. Blacklisting the board in the test wastes contributor's time.
c. Adapting the board's makefile during build system rework takes time and makes
the reworks harder.
c.1. Add to that that the changes are most of the time not even tested on the board
because of (1). Look at the github issues/PRs and you will see it.
d. Developers usually stick to the lowest common denominator. Issue (2) sets this
denominator unacceptably low.
MIPS platform in general
========================
In commits I will address general issues in the MIPS platform and why it should all
be removed.

- Update DMA to use new vendor headers
- Update send to use iolist. It looks like the packet headers are now transfered as seperate iolist entries which results in the eth periph sending each header as own packet. To fix this a rather ugly workaround is used where the whole iolist content is first copied to a static buffer. This will be fixed soon in another commit

The toolchain variables were overwritten anyway in
`makefiles/toolchain/gnu|llvm.inc.mk` as they use `=` affectation,
except for `OBJCOPY`.
On my Linux ubuntu bionic machine, the only difference for `info-build`
with both gnu/llvm and also with LTO=1
is that it now uses an absolute path for `objcopy`
-OBJCOPY: objcopy
+OBJCOPY: /usr/bin/objcopy
On the OSX 17.7.0 I tested, there was no difference.

This variable is only used for the term recipe (and maybe for flashing). They
should not be evaluated if they are not needed and the user should not see a
warning that the port is not set if he does not use port (for example in make

`makefiles/tools/serial.inc.mk` and some boards Makefile.include.
Their value does not need to be exported.

kinetis: save 'wdog-disable.bin' binary in the repository
...
Keep the compiled '.bin' file to remove the need to compile it when
flashing. This remove the need to have the toolchain when flashing so
allow compiling and flashing with `BUILD_IN_DOCKER=1` without a local
toolchain.
Even if it ends up storing a binary, the file is only 34 bytes.

The currently supported SAM0 MCUs (samd21, saml21, saml1x) share the same
Timer peripheral, yet each of them carries it's own copy of the Timer
driver.
This introduces a new timer driver that is common for all sam0 MCUs and
uses structs for configuration instead of defines.

When it was running 'hello-world', the board could not be flashed through
JLinkExe anymore and only through the drag and drop support.
By resetting the board before doing 'loadbin' it can now be flashed
properly.

kinetis: move filtering-out periph_hwrng in cpu/kinetis

This removes doing `filter-out periph_hwrng, $(FEATURES_PROVIDED)`
after processing `cpu/$(CPU)/Makefile.features`.
The current solution is a HACK as `CPU_MODEL` is currently not available
at that moment but will be in the near future.
It will allow always including `cpu/$(CPU)/Makefile.features` after
`boards/$(BOARD)/Makefile.features`.
It is a part of moving `CPU/CPU_MODEL` definitions to `Makefile.features`.

Moving atmega_stdio_init() to cpu_init() just before periph_init() guarantees
that stdio is available to allow DEBUG() in periph_init(). This also helps to
unify the boot up process of ATmega boards and de-duplicates the stdio init from
board_init().

msba2: compile 'lpc2k_pgm' when flashing

Add a rule to build `lpc2k_pgm` when flashing.
It is only compiled if it is using the one in `tools`.
If overwritten to `lpc2k_pgm` if it should be taken from the path,
it is not compiled.
The compilation is still done in `boards/common/msba2/tools` as it was
the case before and this commit does not address this.

boards: Add support for the Arduino Nano

The Arduino Nano board is the cheapest member of the Arduino family and used the
same MCU as the Arduino Uno. It differs in the form factor (the Nano is much
smaller), it uses an integrated FT232RL TTL adapter instead of an ATmega16u2 to
provide access to the serial console via USB, and it uses a different
bootloader (which occupies 2 KiB of the 32 KiB flash instead of 0.5 KiB occupied
on the Arduino Uno). This commit mostly copy pastes code from the Arduino Uno.

The most simple motor driver type is used here (MOTOR_DRIVER_1_DIR) which
needs only one pin to work.

native_motor_driver_qdec_simulation() callback is called each time
motor_set() from motor_driver driver is called.
It set associated qdec value to the PWM duty cycle.
QDEC values are stored in qdecs_value[] array in the order motors
are described in board.h.
Then it is needed to configure in first motors that needs the QDEC in
board.h.

The CPU variable in the boards Makefile.include file already contains the target
CPU, so there is no reason to provide it in each board again as avrdude flag.

boards/nz32-sc151: do not blindly set TERMFLAGS

The board should not globally set `TERMFLAGS` without a RIOT_TERMINAL
condition.
Also the board does not export uart through usb and thus needs an uart
converter connected. Other boards in the same situation do not need
to set `TERMPROG` without a valid baudrate. So remove the definition
anyway.
This also removes the export of TERM* variables and should make it work
with the other RIOT_TERMINAL.

The periph_dma is not pulled in automatically for all applications.
Applications willing to use the configured peripherals with periph_dma
for a given board will have to include the feature explictly in their
Makefile.
It was causing unrelated issues as threads got de-scheduled while
calling printf and this was not handled properly in tests at that
moment.

Currently only tested boards provide the feature riotboot.
Potentially all boards embeding a cortex-m0+/3/4/7 are
able to have riotboot as a feature, but other dependencies
need to be met, e.g. usage of cortexm.ld linker script,
double initialisation of cpu_init(), etc. See doc in
bootloaders/riotboot.

This commit fixes configuration problems when trying to use i2c pins that need to be remapped.
All B8 and B9 pins for STM32F1 need to be remapped, so a check is done if the remappable pins are selected.

With the canned recipe for flashing, flash dependencies should be
added to FLASHDEPS, instead of writing `flash: dependencies`. This

This commit add the connect_assert_srst which requires the SRST to be asserted before trying to connect.
Certain firmwares will cause it to get to a state where flashing fails and the reset button must be pushed at the correct time to recover.

Boards should not set PORT and should not have code conditional on
PORT as that causes PORT to be evaluated and the build to fail even
if this varible is not needed.
Exporting has the same effect.
This fixes the MSBA2 board by declaring PORT_LINUX and PORT_DARWIN
instead.

The STM32F103C8 secretly comes with 128KiB flash instead of 64KiB. Still, only
64KiB of it are tested and guaranteed to work. However, most of the times the
whole 128KiB flash works just fine. In the BluePill documentation this fact is
already documented and by using
$ make BOARD=bluepill CPU_MODEL=stm32f103cb
the whole 128 KiB can be used by RIOT. When using this hack routinely, it easier
to use environment variables instead. But allowing to overwrite CPU_MODEL via
environment variables seems to be a bad thing, as it is easy to forget to clear
that environment variable when changing the BOARD variable.
This commit introduces the new STM32F103C8_FLASH_HACK variable, which unlocks
the 128KiB FLASH when set to "1". The BluePill documentation has been updated
accordingly.

cpu/esp32: allow explicit ESP32 crystal freq configuration

Some ESP32 boards (like my SparkFun ESP32 Thing) have a main clock
crystal that runs at 26MHz, not 40MHz. RIOT appears to assume 40MHz.
The mismatch causes the UART to not sync properly, resulting in
garbage written to the terminal instead of log output.
I’ve added:
* A new board configuration constant ESP32_XTAL_FREQ that defaults
to 40, but can be overridden by a board def or at build time to
force a specific value (i.e. 26).
* Some code spliced into system_clk_init() to check this constant and
call rtc_clk_init() to set the correct frequency.
* A copy of the rtf_clk_init() function from the ESP-IDF sources.

The board should process its CPU features.
This has the consequence to make 'periph_pm' visible to the board.
I also removed the duplicate FEATURES_PROVIDED from the board defined in
the cpu.

Remove duplicate handling of `periph_rtc` dependency to `periph_rtt` now
declared in the cpu.
Also the makefiles should not parse FEATURES_REQUIRED as it does not
guarantee anything as features could also be requested by FEATURES_OPTIONAL.
In practice it should parse it using `USEMODULE`.

This change was missed when the cpu 'mips32r2_generic' was separated
from 'mips32r2_common'. The target file already includes 'mips32r2_common'.

RF_SWITCH_2_4_GHZ_ON should turn PD4 low not high to active the 2.4 GHz radio.
The same goes for RF_SWITCH_SUB_GHZ_ON on PD3.
